Letter of Authorization
I authorized Media One Online, LLC to provide
services I selected in E-Commerce
Plans , for my
location. I understand that there is a monthly service charge fee
based on the hosting service I selected, that will be charge to my
ILD page of your local phone bill. This SSA (Subscriber Service
Agreement) / Letter of Authorization (L.O.A) gives Media One Online,
LLC authorization to bill the ILD page of your local phone bill, to
facilitate this order, and provide Internet access & Web
Hosting/Design service. Further, I agree to pay all toll charges by
the due dated stated in the invoice. Failure to do so may result in
discontinuation of service. I also agree to pay a reasonable
attorney fee and/or collections fee if the account is turned over to
an attorney or collections agency. Cancellation can be made by
calling 1-888-292.1829.
*Are you 18 years of age and duly authorized
by the telephone account owner to make changes to and/or incur
charges on the telephone account.
